- Certified original middle school transcript.
- Central Admission Form.
- Medical Examination Form.
Personal Documents
- Unified National ID Card.
- Residence Card.
- Six (6) recent passport-size photos.
Additional Requirements
- Legal declaration.
- Sponsor’s documents (if required).
- Documents for special admission channels, if applicable.
Note:
After completing the above documents, the student should visit the Registration and Student Affairs Division at the College of Health and Medical Technologies / Al-Dour to complete the registration process.
